Summary of Benefits for Production Employees
You have an opportunity to become part of a great plan of health insurance, short-term disability, and life insurance. You will become eligible for the insurance the first of the following month after the completion of five (5) months of employment. A full-time employee must be regularly scheduled to work at least 32 hours per week with the Company.
Health Insurance:- $20.00/weekly premium for single or family coverage
- Blue Cross/Blue Shield of MN is our provider
- PPO In-Network 90/10 after deductible met
- Out of Network 60/40 after deductible met
- Deductibles: $300 Single/$600 Family
- Life Time Maximum $1,000,000.00
- Out of Pocket Maximum In-Network: $1300 Single/$2600 Family
- Non-emergency room charges are not covered by the plan
- Preventitive care coverage 90%, up to $250 per visit
- Prenatal 100% coverage after deductible if enrolled in Healthy Start Prenatal support; otherwise 90% coverage
- Employee Assistance Program for mental health, etc…90% coverage after deductible up to 40 days
Prescription Drug Coverage
- Express Scripts is our prescription drug provider
- 25% of prescription cost (max $20 generic; max $30 preferred brand; max $40 non-preferred brand)
- 25% of prescription cost (max $20 generic; max $30 preferred brand; max $40 non-preferred brand)
- Full cost for lifestyle drugs
- Mail Order/Long Term Prescription $20.00 max generic, $40 max preferred brand; and $60 max non-preferred brand.
Dental Coverage
- Delta Dental is our dental provider
- $25.00 annual deductible
- 100% coverage for preventive dental care; 80% coverage for basic services & 50% major services (all in-network coverages)
- Maximum benefit is $1500.00 per individual per year (excludes orthodontic)
Short Term Disability
- $240.00 per week
- Pay begins 1st day of accident and 4th day of illness. Begins from 1st day visit/treatment by physician
- Maximum 18 week benefit
Life & Accidental Death Insurance
- $6,000.00 lump sum payment for qualifying events
Pension
- $20.00 credited per service year
- Vested 100% after 5 years
401(K)
- Employees may contribute after 1 full month of employment
