Summary of Benefits for Production Employees

You have an opportunity to become part of a great health insurance, short-term disability and life insurance plan. Full time employees will become eligible for insurance coverage on the 1st of the month following a three (3) month waiting period from the date of hire.

Health Insurance:
  • PPO In-Network 90/10 after deductible is met
  • Out of Network 70/30 after deductible is met
  • Office visit copay (In-Network): $15
  • Deductibles: $300 Single/$600 Family
  • Out of Pocket Maximum for PPO: $1300 Single/$2600 Family
  • Out of Pocket Maximum for Non PPO: $3300 Single/$6600 Family
  • Life Time Maximum: $1,000,000
  • Prenatal Healthy Start Program
  • Employee Assistance Program for mental health, etc.

Prescription Drug Coverage
  • Retail Pharmacy Copay: 25% up to maximum of $20, 31 day supply
  • Mail Services Copay: $20, 90 day supply

Dental Coverage
  • PPO In-Network 80/20
  • $1000 annual limit

Short Term Disability
  • $165 per week
  • Pay begins 1 st day of accident and 4 th day of illness
  • Maximum 18 week benefit

Life & Accidental Death Insurance
  • $5000.00 lump sum payment for qualifying events

Pension
  • $15.00 credited per year of service
  • Vested 100% after 5 years

401(K)
  • Employees may contribute after their probationary period ends
